When you’re trying to conceive, navigating the wealth of information and advice can be overwhelming. Among the myriad suggestions, one that occasionally comes up is the use of Mucinex, an over-the-counter medication known for its use in relieving chest congestion. But can it really help with fertility? Let’s take an objective look at the evidence.
How could Mucinex improve fertility? The theory
Mucinex contains guaifenesin, an expectorant that works by thinning mucus in the lungs and throat, making it easier to expel. The theory behind using Mucinex for fertility is that guaifenesin might also thin cervical mucus. This could potentially make it easier for sperm to travel through the cervix and reach the egg.
The research
The idea that Mucinex could aid fertility stems from a small study conducted in 1982 by Check et al. (Et al. is a scientific way to abbreviate a reference to paper that has multiple authors.) This study involved 40 couples who had been trying to conceive for at least 10 months and were found to have “hostile” cervical mucus. In this case, hostile is referring to thicker mucus that makes it more difficult for the sperm to travel through. The women took 200 milligrams of guaifenesin three times daily from day five of their menstrual cycle until ovulation.
Results showed that more than half of the participants experienced an improvement in the quality of their cervical mucus, and 40% of the women became pregnant after taking Mucinex. While these findings are intriguing, experts caution against relying on this study as definitive proof due to its limitations.
What does this mean?
Before you rush to get a box of Mucinex, let’s take a deeper look at the study. Unfortunately, the study was not well-designed. It lacked a control group, which means a group who did nothing, so we could compare what difference (if any) the Mucinex made. This makes it difficult to attribute the pregnancies solely to Mucinex. We simply don’t know if Mucinex was the reason for the pregnancies.
This means there really is no good evidence that using medications to thin the mucus (e.g., guaifenesin) improves fertility. There are no other studies examining the effect of guaifenesin (the active ingredient in Mucinex). We need several, quality studies, ideally double-blind, randomized controlled trials, to conclude that Mucinex is effective for improving fertility.
Even if it did thin cervical mucus, to enhance your chances of conception, thick cervical mucus would need to be the reason you’re not getting pregnant. For many couples, thick cervical mucus is not the primary issue. Think of it like a car. If your engine is sputtering and the car won’t drive, changing the tires isn’t going to fix the issue.
But what about so-and-so who got pregnant using Mucinex?
You may know someone who used Mucinex and got pregnant. So, doesn’t that mean it works? Most reported successes on social media are likely coincidental or due to the use of other fertility treatments in combination with Mucinex.
Moreover, a spokesperson for Reckitt, the company that manufactures Mucinex, has emphasized that Mucinex should “only be used as intended and in line with usage instructions,” noting that using it for infertility is an off-label use. Off label means it’s not approved by the FDA.
Safety Considerations
While Mucinex is generally considered safe and has a low risk of serious side effects, it is crucial to use it appropriately. Guaifenesin can be found in combination with other medications, such as pseudoephedrine, which is intended for nasal congestion. Taking medications outside of their approved indications or without acknowledging all active ingredients can be harmful. Some people may experience nausea or headaches.

Conclusion
While the idea of using Mucinex to aid fertility is based on the concept of thinning cervical mucus, there is no strong scientific evidence to support its effectiveness.
